Case Evaluation
Before you Start…
I’m sorry you’re dealing with something this serious. Motorcycle crashes, bicycle collisions, and wrongful death cases turn life upside down and often feel overwhelming.
I personally review every submission — not a call center, not a gatekeeper, and not a marketing team. Just me. My firm is selective. We take on fewer cases so we can focus on cases where the facts, injuries, and circumstances allow us to make a real difference.
This case evaluation form gives me the information I need to decide whether we’re the right firm for you and whether I believe your case can be handled responsibly and effectively. If your situation appears to be a potential fit, I’ll follow up to discuss next steps. If not, I’ll do my best to point you toward helpful resources.
Please answer each question completely and honestly. Cases involving serious injury require cooperation, documentation, and a shared commitment to accuracy. Incomplete or inaccurate information can delay or prevent review.
Completion of this form is required before any consultation can be scheduled. Submitting this form does not create an attorney‑client relationship. Everything you share here will be kept confidential.
